Boston PCN is a network of six practices that collaborate to improve patient health.

To support them in delivering additional services there is a PCN Management Team.

The role of the PCN Administrator is to provide efficient secretarial and administrative support to the PCN Clinical Director and the PCN Management Team.

A Cancer Care Coordinator is a member of the Primary Care Team here at your GP surgery who can support you throughout your treatment and beyond. Your GP Practice will make you known to our Cancer Care Coordinator team who will get in touch with you to talk about how best to help you, especially following diagnosis.

Care Coordinators – work with people who need extra support to navigate the health care system, helping to connect them to the right services or professionals at the right time. They act as a central point of contact to ensure support is available to help people manage their care.

General Practice Assistants (GPAs) - form part of a multi-disciplinary team within primary care, and provide a support role, carrying out administrative tasks, combined in some areas with basic clinical duties, helping to free up GPs time and contribute to the smooth running of appointments, improving patients experience in the surgery.

Occupational Therapists - empower patients to make improvements in day-to-day life and can discuss and help arrange a variety of aids. They support a range of people from frailty/falls to mental health.

Clinical Pharmacists – answer your questions about medications and how to get the best results from them. They support the Practices with medication reviews and safety monitoring. Some of our clinical pharmacists will also work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.

Pharmacy Technicians – liaise with Clinical and Community Pharmacists, GP Practices, and hospitals to ensure you get the correct medications at the right time. They review medications and are available to help you understand how to use your medications safely.

Physiotherapists – are available to diagnose and treat muscular and joint conditions. They can advise on managing your condition or refer to specialist services if needed.

Our First Contact Physiotherapists are:

Project support officers provides support to management teams for one or more programmes or projects. Their role is likely to include: setting up and maintaining project documentation. producing project reports.
