Clinical Pharmacists – answer your questions about medications and how to get the best results from them. They support the Practices with medication reviews and safety monitoring. Some of our clinical pharmacists will also work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.

 ​​​​​Previous experience working across multiple healthcare settings. This includes secondary care at Leeds Teaching Hospitals and Nottingham University Hospitals, third-party clinical service provision of chronic disease management clinics in primary care across the UK, and most recently commissioning experience as a Medicines Management Pharmacist with Optum. Joined Boston PCN as Lead Pharmacist in April 2020.

As Lead Pharmacist for the wider PCN team, we are keen to work collaboratively with all organisations on prescribing and medicines related initiatives.

General Practice Assistants (GPAs) - form part of a multi-disciplinary team within primary care, and provide a support role, carrying out administrative tasks, combined in some areas with basic clinical duties, helping to free up GPs time and contribute to the smooth running of appointments, improving patients experience in the surgery.

Health and wellbeing coaches (H&WBC) - support people to increase their ability to self-manage, motivation levels and commitment to change their lifestyle. They are experts in behaviour change and focus on improving health related outcomes by working with people to set personalised goals and change their behaviours. They work with people with physical and/or mental health conditions and those at risk of developing them.

Jenny Streather - Occupational Therapist & Clinical Lead for Care Homes 

I have worked across a number of NHS and Charitable Organisations throughout my career.  Prior to qualifying I worked as a Therapy Assistant and then Assistant Practitioner in Occupational Therapy Services in Acute and Community Services in Liverpool.  After Qualifying from Salford University I began a rotation at Queen Elizabeth Hospital in Kings Lynn – working across a number of specialities.  After just over 2 years I Specialised in Oncology, Haematology and Palliative Care – delivering a Therapy Service across the Ward, Outpatient and Community settings – here I was able to work within a very Integrated Team.  I moved to Lincolnshire in 2015 and began working for St Barnabas Hospice.  Here I enjoyed 6 years of promoting the value of Occupational Therapy and Physiotherapy within a Hospice setting which was traditionally a medical and nursing led service.  Over my time there I was able to grow and develop the Team.  In February 2022 I joined the Boston PCN as the Lead Occupational Therapist.  My focus has been understanding and articulating the need for fair access to Therapy Services within Primary Care particularly in Care Homes.

Statement – To promote the value and potential of Occupational Therapy in Primary Care – whilst identifying and developing Therapy Services to support those people falling through the net of traditional Pathways and referral criteria to have access to Occupational Therapy.

Mark Yego - Occupational Therapist & Clinical Lead for Wellbeing Team

Physiotherapists – are available to diagnose and treat muscular and joint conditions. They can advise on managing your condition or refer to specialist services if needed.

Our First Contact Physiotherapists are:

Pharmacy Technicians – liaise with Clinical and Community Pharmacists, GP Practices, and hospitals to ensure you get the correct medications at the right time. They review medications and are available to help you understand how to use your medications safely.
